5. A 22 year old male was involved in an RTA. He had been knocked off his bicycle and had suffered internal bleeding. He was taken to a hospital which was very busy. He urgently needed a transfusion and due to staff shortages, this was given by an inexperienced nurse who wasn’t sure exactly how to set up the infusion line. The blood was seen to be flowing to the patient. The patient almost immediately developed severe shortness of breath and then collapsed.
